Once the initial requirements for a bingo license application have been met, the application for a new bingo license must be completed. This requires information about all members of the organization applying for the license as well as general bank information.

A second questionnaire in regards to the bingo facility is required as part of the application.
Release of Information for bingo chairperson
The chairperson of the organization seeking a bingo license must be subject to a background check by Code Enforcement. A release of information must be signed and submitted with the application for that background check to be processed.
